Advance Care Planning

Advance Care Planning is a process of planning for future medical care in case you are unable to make your own decisions. It is a continual process and not merely a document or isolated event. Advance Care Planning assists you in preparing for a sudden unexpected illness from which you expect to recover, as well as the dying process and ultimately death. The booklet below includes all the legal forms as well as information necessary to ensure that your wishes will be carried out, based on your values and beliefs.

La Planificacisn del Cuido Avanzado es un proceso de planificar para el cuido midico del futuro en caso de que no pueda tomar sus propias decisiones. Es un proceso contmnuo y no solo un documento o evento isolado. La Planificacisn del Cuido Avanzado le asiste a preparar por una enfermedad inesperada por la cual se espera recuperar, tanto como el proceso de morir y, zltimamente, la muerte.
