Frequently Asked Questions About Primary Care

Are there different types of routine medical exams?

Yes, depending on the age and need of the patient, types of visits include:

  • Health Review: A general health and medical history assessment, involving a physical exam of the whole body and major systems (heart, lungs, etc.), as well as a general medical history. Routine tests may be performed including those to check cholesterol levels and blood pressure.
  • Well Child Checkups: A well child checkup includes an assessment of your child’s growth, development, diet and nutritional requirements, as well as his/her physical, social and emotional health.
  • Medicare Wellness Visit: A health risk assessment for eligible Medicare patients to review current health and risk factors and develop or update a personal prevention plan. The exam is fully covered by Medicare once every 12 months. 

What should I bring to my appointment?

Please bring:

  • Your insurance card and photo identification
  • A comprehensive list of the medications you are taking, including over-the-counter medicines, vitamins, and herbal treatments
  • A list of all of the symptoms you may be currently  having or have recently experienced
  • A list of any questions you’d like to ask
  • Any forms that may need to be filled out by your provider for work, school, sports or camp

If you anticipate that you may need to discuss sensitive or detailed medical information during your appointment, you may also wish to bring a family member or friend  to your appointment for support.

How long will I have to wait for an appointment?

The Lifetime Health Medical Group practices are available for same day appointments for sudden illness or acute needs. Sick visits can usually be accommodated on the same day that you call. Most new patients (adult or pediatric) can be seen within 1-3 days if necessary, or if there are no urgent needs, we will schedule a time that is convenient for you. A pediatric patient can get a "well child checkup" appointment normally within 1-2 weeks.

What if I can’t make my scheduled appointment?

At Lifetime Health Medical Group, we strive to make appointment times as convenient as possible. If you have scheduled an appointment and are unable to make your visit, please call as soon as possible to cancel your appointment.

What if my physician is not available?

If your primary care physician is not available, one of our other experienced providers will be available to care for you.

When should I schedule my newborn’s first appointment?

Your child’s first visit should be within the first week of leaving the hospital. During your baby's first year, please schedule visits at 2 weeks, 2, 4, 6, 9 and 12 months. A child should also be seen at 15, 18 and 24 months of age. After age 2, we recommend well child checkups once a year.

What if I have to see a specialist?

Your primary care physician and health care team will work with you to identify specialists who will meet your needs.

What should I do if I have a medical emergency?

If you are experiencing a true medical emergency, please call 911 and seek immediate medical attention. If you have an urgent need after regular office hours, call your doctor’s office and trained nurses or an on-call physician will assist you. Depending upon your need, you will receive home care advice, be offered a next-day appointment, or be directed to an urgent care or emergency center. Urgent Care by Lifetime Health is available for urgent, non-life threatening illness or injury morning through evening, weekdays, weekends and holidays.

Can I email my physician?

Our patients can send secure online messages to their physician and much more through MyLifetimeHealth, the secure patient portal from NextMD. Enroll at your practice and/or visit the MyLifetimeHealth page for more information.

How do I request a copy of my medical records?

Patients may request a copy of their medical record through MyLifetimeHealth or contact their Lifetime Health office to request a copy.

Will my medical information be kept confidential?

At Lifetime Health Medical Group we safeguard all patient information. We strictly adhere to all health care privacy laws, and only the providers involved in your care or caretakers you designate will have access to your information.
